Considering how deeply the Twelve Steps approach is embedded in rehab and recovery programs, it is surprising that there is little literature on the success rates of teenagers who follow the Twelve Steps. The studies that are available tend to focus on the adolescents who finished a treatment program, leaving out those who left the treatment centers. Of those studied, 30 to 40 percent were still abstinent when checked at various points throughout the study. Steve Sussman reported these findings in Evaluation & the Health Professions, stating his belief that the figures were overly optimistic because of the dropout factor.

Teenagers who continue to attend Narcotics Anonymous or Alcoholics Anonymous are two to three times more likely to remain clean and sober, especially if they attend two meetings a week. According to Anne Fletcher in Inside Rehab, most teenagers do not attend a Twelve Steps meeting, and if they do, they tend to drop out over time. One study shows that of those who were attending meetings at the conclusion of their treatment programs, only one-third remained over a longer period of time. In general, the dropout rate for teenagers is high. Those who attend any treatment program show a dropout rate of about 50 percent during the first ninety days. Adolescents who have an adult take them to their first meeting and introduce them to others in the group tend to have a higher rate of retention.
